Abstract

Mechanical ventilation is a life-saving intervention but remains a primary driver of secondary lung damage, known as Ventilator-Induced Lung Injury (VILI). While traditional lung-protective strategies—specifically low tidal volume (V_T) and plateau pressure limiting—have been the standard of care, mortality rates for Acute Respiratory Distress Syndrome (ARDS) remain high. Recent research has shifted the focus from static volume/pressure limits to dynamic, individualized metrics that account for the lung's biological and physical heterogeneity.


Objective: This study reviews and evaluates emerging strategies for VILI mitigation, moving beyond "one-size-fits-all" protocols toward personalized, physiology-guided respiratory support.
