GYNAECOLOGIC SURGERY AND MINIMALLY INVASIVE TECHNIQUES: IMPROVING PATIENT OUTCOMES IN ONCOLOGY

Abstract

The gynaecologic cancers are a significant health problem worldwide, and surgery is one of the principal modalities of treatment used in the treatment of ovarian, cervical, and endometrial cancers. Minimally invasive surgery (MIS) has been gaining ground in gynaecologic oncology over the last few years, because it has the potential of enhancing the patient's peri- and postoperative experience. The objective of this study was to assess the impact of minimally invasive surgery on the improvement of the patient's recovery, the decrease in surgical problems, and the oncologic effectiveness in the treatment of gynaecologic cancer. A narrative review methodology was used to review and synthesise existing publications concerning minimally invasive gynaecologic oncology procedures (MOPs), robotic-assisted surgery, and the use of enhanced recovery protocols (ERPs) and emerging surgical innovations. To assess the clinical effects of minimally invasive methods on oncology patients, relevant peer-reviewed studies were reviewed.  The literature reviewed showed that the minimally invasive surgical techniques resulted in less intraoperative blood loss, shorter hospital stay, decreased postoperative pain, faster recovery, and fewer complications as compared to open surgery. The use of robotically assisted procedures and image-guided interventions further improved the accuracy of surgical procedures and safety during interventions in complex oncologic cases. Furthermore, using Enhanced Recovery After Surgery (ERAS) protocols helped to enhance post-operative recovery and opioid consumption in gynaecologic oncology patients. In general, MIS has proven to be a very successful and patient-centred method in gynaecologic oncology. New and future developments in the surgical field, as well as the integration of several disciplines into the perioperative approach, should lead to even better surgical outcomes and quality of care in gynaecologic cancer management.
