A COMPARATIVE STUDY OF ADIPOKINE PROFILES IN DIABETIC AND NON-DIABETIC INDIVIDUALS WITH A HISTORY OF SUBSTANCE ABUSE

Abstract
Background:Adipokines play crucial roles in glucose homeostasis and metabolic regulation. Theconcurrent presence of diabetes and substance abuse may create unique metabolic perturbationsaffecting adipokine profiles. This study aimed to compare adipokine levels between diabetic andnon-diabetic individuals with substance abuse history.
Methods:A comparative cross-sectional study was conducted at Maa Vindhyawasini AutonomousState Medical College, Mirzapur, from January to August 2025. One hundred participants withdocumented substance abuse history were enrolled: 50 with type 2 diabetes and 50 non-diabeticcontrols. Serum adiponectin, leptin, and resistin levels were measured using ELISA. Clinicalparameters, anthropometric measurements, and substance abuse patterns were assessed.
Results:Diabetic participants showed significantly lower adiponectin levels (4.8±2.6 vs 9.2±3.8μg/ml, p<0.001) and higher leptin (14.6±8.4 vs 8.8±5.2 ng/ml, p<0.001) and resistin levels
(18.4±6.8 vs 12.6±4.2 ng/ml, p<0.001) compared to non-diabetic individuals. The leptin-to-adiponectin ratio was 3.2-fold higher in diabetic participants (3.8±2.1 vs 1.2±0.8, p<0.001). Strongcorrelations were observed between adipokine levels and metabolic parameters including BMI,HbA1c, and HOMA-IR. Multiple regression analysis identified diabetes status, BMI, and substanceabuse duration as independent predictors of adipokine alterations.
Conclusion:Individuals with concurrent diabetes and substance abuse demonstrate profoundadipokine dysregulation characterized by severe hypoadiponectinemia and elevated pro-inflammatory adipokines. These findings suggest heightened metabolic risk requiring targetedinterventions and intensive monitoring in this vulnerable population.
